hi this is Melissa your Social Security lawyer and I am here today to walk you through the SSA 3369bk the work history report this is a form you're going to fill out at least once during your application for Social Security disability and it's a pain in the neck yeah if you're someone who's had a lot of jobs it's a ton of work and even if you're somebody who's only had one job or a couple of different types of jobs uh it's just something that's pretty frustrating and I think confusing for a lay person so let's go over it on the first page you're going to list all the jobs that you've had for the last 15 years um and so basically you need to stretch your memory for everything that you've done since 15 years before the date of your application uh and then here I'm looking at job title number one and so basically you're going to go through and for every job that you've held you're going to fill out all of this information so you'll need the job title and in that case I encourage you to use something that's descriptive so rather than my job title associate attorney department head for Social Security Department I would write attorney or Social Security attorney because that describes what I do rather than necessarily describing my role within the firm or whatever job titles I have to make my resume look more fancy uh you'll put your rate of pay then I I encourage people to use their most recent rate of pay or as close as you can remember for hours per day days per week again you're shooting for the average if you always did over time put it in there if you did over time once a year you can skip it and just write 40 hours in terms of describing the job what do you do all day for that section we're not looking for your resume answer um you're not trying to impress anybody you're trying to explain the nature of your job Duty so for me as an attorney I might write communicating with clients attending hearings writing legal memoranda doing legal research uh and basically describing both the mentally hard things I do or you know describing the mental duties of my job particularly as well as describing the physical Duty so for me I might write carrying clients files doing filing um walking to and from courthouse so uh particularly for jobs that are a little bit more blue-collar where you're doing more physical things it's important to put the physical tasks that you did in that section uh the next section in this job did use machine tools or equipment uh here I would say you know if you're using a computer if you're using a cash register that's going to count as a machine or tool that helps you do your job uh obviously
