To complete this form, you must provide the following information: 1) Name of owner's name you are talking to and date of accident; 2) Contact details of victim and the victim's family; 3) Current age of victim; 4) Victim's name and identification information; 5) Victim's family (the Victim's and Police Families); 6) Victim's name and victim's age; and 7) The name and identification number. (I was working in the field, and I only included the Victim's name and ID number for the MV-104 form, but you can still see the victim's name and ID numbers if both were taken as required.) It is important to note that the “victim” number was in my driver's license number instead of the victim's. Because the MV-104 form was so short, I chose to include the Victim's age and other information instead of the Victim's address. If the accident is an accident in which the motorist had to travel to and passed the intersection and hit another vehicle that had not struck the victim. These events may cause property damage to other vehicles on my property, and you can do the same with the “victims”. So, for instance if you were driving near a bus stop on the side with the vehicle that had not struck you, you may have the victim's name on it so that you can share this information with someone. On other occasions, you may own personal property you may not need the information about. You may want to have it placed in a safe place with clear locks. NY State Vehicle and Traffic Law — General Information What is an accident report? An accident report is any reporting required under the following NY law if your vehicle is going through an intersection or is blocked outside: The New York State Code requires a driver of a motor vehicle with at least two occupants to report the accident. You must file the accident Report or you have to pay a 45 to receive a 30 penalty. You must provide your vehicle's address, if anyone was at the stop or a motorcade was on board the vehicle, if anyone was stopped or stopped by at least a few officers, and if anyone was in violation of the law, you can file a report. In the event that there is a fire or other hazardous incident involving a person who is under 18 years of age, the driver must report to the police department, fire station, or hospital in New York where such person is under the age of 18.

The DMV may give you an estimate if you have filed any of your accidents. Do not assume that all the accidents happen in a single year. Insurance companies are not obligated to have any records of accidents. You should check with the DMV on your claims using the Form 990 from the Massachusetts Highway Patrol. Many accidents are treated as minor cases when they have been documented for ten years. You must report each case separately, unless you are responsible solely for reporting them. You are also responsible for filing all papers that you report and the records that you file. However, some accidents are treated as minor if the injury or the cause is a major injury, or if the injury was fatal, and your insurance company is not obligated to post their medical records. Reporting Cases If you are responsible for reporting to an auto accident or crash insurance company, you need to report both accidents to the Department of Motor Vehicles. For instance, if your accident results from a wreck, the DMV should post that accident records to the Massachusetts Department of Motor Vehicles. If your accident does not result in a claim, the DMV will ask you to fill out the DMV Form 990 to make sure that you are 100% sure you are claiming the insurance for the wrong vehicle. If you have received insurance from a local insurance company, the DMV will provide you with an initial statement about the insurance in that company's insurance application and may ask you to fill out an original Massachusetts State Highway Patrol Form (Form 990) if you have had a major accident.

The short answer is no. Insurance companies do not report accidents to the DMV. But that doesnt mean the DMV isnt aware of an accident you were involved in. Accidents that generate a police report are filed with the DMV. In some states, however, police reports arent necessary generated for minor accidents.

The DMV also makes certain modifications to their system to detect violations. For instance, vehicles with windshield wipers can be registered for two years with no change on the windshield. If you do not file a report with the DMV, or if a vehicle is in a condition requiring a permanent change in the vehicle's condition within the year, you are responsible for the money spent on that adjustment for the year, even if the vehicle is being sold. Your California insurance company or your local law enforcement agency can help you make a payment, if any, based on the amount of the change. It is important that you file a claim on your car's license plate at the DMV; you will have to pay a few fees before your car becomes permanently registered. The California Vehicle Registration Commission doesn't usually approve of new plates because it is not used to prove that an owner has been convicted of a motor vehicle accident. However, it's common that people with a license plate are charged a different fee. If your California insurance company doesn't have a fee on your plate, you might not be able to get another plate in case of an accident. Insurance companies must set the fee according to the vehicle's performance characteristics. Many counties also have a 150 per plate fee for every vehicle in California. The DMV will not provide an SR-1 form for all types of crashes and suspensions. Check your car for any problems that may have happened or that may not be resolved until your DMV issues your vehicle. DMV is committed to the best interests of all customers. This DMV form can help you to determine if your car has been repaired as well as what parts may have changed (if any). Your car may have problems with your vehicle's transmission, transmission-related safety components including suspension components, suspension pads and wheels. Your car may be damaged by your vehicle's rear-facing window system (HRD). You must be in good standing. FULL REGISTER The DMV is a licensed and registered dealer who offers your vehicle for free or at no added charge.

Reporting a Collision to DMV When you have a collision, report it to DMV using an SR 1 form. You or your insurance agent, broker, or legal representative must complete an SR 1 report and send it to DMV within 10 days if someone is injured no matter how minor the injury or killed, or property damage is over 1000.

A second statute requires a person under a legal duty under California's road traffic law to comply with the conditions set forth in any statute as long as the person has committed no driving or is in violation of those regulations. California's first statute sets a minimum age of 15 for a driver under such age to take the road and to pay a license suspension of at least 24 months. The new statute only applies to a person under the age of 15 in California and applies to persons 25 and older. In an attempt to enforce the new California driver's license law, the Department of Public Safety announced Tuesday night the enforcement of the law on January 1 that will require that all vehicles at intersections on streets with a population of more than 1.5 million who are under 20 years of age must have a valid California driver license by December 31, 2011, unless the California DMV or DMV Security Department gives any reason. State legislators passed the driver rights law in 2010 and 2012 at their annual convention. The new driver rights law was introduced by then state Sen. George Garcia, R-Berkeley. On Oct. 4, the legislature passed a measure to give those under the age of 16 more time to comply with the new driver rights law even if driving on their own property and on school transportation is not permitted. Garcia added he supports a driver education program and also wants to build and equip schools with drivers who will assist students in passing high school exams and passing exams in their primary education. The new law requires that drivers with valid CA driver licenses and a parent's permit, regardless of age, not drive on a California road before noon on February 8, 2012, to give DMV 60 days to complete the course of the course.

No other claim may be brought. If the car is still on the roads the DMV may take it for a full test or, if it is no longer operating, put it off a test for compliance. How to report a collision? A car can be considered inactivity after it has been stopped for a stop on a road if that stop had been recorded as an event where the car's speed exceeded 100 km/h or exceeded 200 km/h. If the car is operating on its own and is not stopping but has been stopped for a stop on a road then a report could be made by phone or video call, by mail, or as part of a registration statement. If a registration statement for an event is required to be signed by one of the following: (1) A California state agency (such as California Motor Vehicle Commission). (2) A California state agency (for any event involving motor vehicle maintenance or registration) that is part of a nonprofit organization designated by Proposition 64 in lieu of state law or that has not yet attained its mission or objective. Such State agency is required to identify the actual cause and cause of the accident before making an entry to the State Highway Patrol. The California Highway Patrol will inspect the car at an inspection site designated by the operator for all motor vehicles and shall inspect the car for all damage to any non-residents or property they enter.
