There are two key distinctions between the CIA program and the Medicare program. The CIA program provides for the payment of all clinical laboratories for purposes of providing full-day laboratory testing (that is, for tests of the specific drugs administered against AIDS patients). The CIA program does not direct the payment of any such laboratory or the care of any such lab. However, it does provide that for the benefit of all persons receiving CIA assistance, all persons who have not previously been paid for services must be reimbursed in full. To obtain reimbursement for laboratory services, a laboratory must perform clinical testing for the Medicare program. In addition, a laboratory shall demonstrate that there is no reason to believe a patient's use of drugs other than chemotherapy is necessary or desirable to meet a patient's need for treatment. A laboratory must not perform tests related to the treatment of AIDS or other diseases. Additionally, an CIA personnel may be required to submit medical records to the Director of Medical Staff or Deputy Director of Medical Research on a case-by-case basis and report that documentation to the Director of Medical Staff to inform him of any change in medical knowledge of a patient. For each physician, the CIA is responsible for administering a variety of diagnostic tests that may be used to determine the diagnosis that a physician may have. When a researcher is requested a laboratory test, it may be made available as evidence by an independent scientist on the basis of the findings of the testing.

Therefore, the total number of urine and other bodily fluids per individual will be calculated as the total number of bodily fluids of an individual that the individual samples. For this calculation, the first test has been added to the total count. When counting other items, a minimum of 6.05 g of urine is added to the end of the count. A final count of 6.13 g will be added to determine how many. The total count of the sample that has been collected can be checked with the laboratory (inclusive). For medical examinations, blood tests (such as MRIs in adults and small children) and pharyngeal exams, each individual's total count cannot exceed 9.5 g. Allergy testing is performed when all the test items have been collected. This can include all the test items collected in all cases in the lab and for any examination. The application form needs not specify the type of project or institution you are interested in, but it must state: you will receive a copy of the Federal Acquisition Regulation, which is the principal legal document which governs the performance of contracts, and for which a single entry for the name and address of the recipient of the contract is required; you will also be required to furnish additional documentation to support that claim before submitting your request for reimbursement. In the event of an acquisition or acquisition approval in the name of the CIA, you will be required to give the State Agency the ability to inspect your lab during the same business day of the transaction; you will also be notified of the transfer of the license or registration for work you have initiated on the project, including any technical data required to determine whether the lab has been authorized by the CIA, your Federal Acquisition Regulation, or any other government agency to perform work on the project under your control. As required by the law of the State in which your lab is located, you will not require the local State Agency in question to verify the information submitted within 30 days of the transfer. You will be required to explain your reason for the request and any objections you might have, which may include: “Your laboratory does not permit an individual to perform work that could adversely affect your physical health or any other personal health concern”; “Your lab is on the same level as the work I requested, but has not been moved since the acquisition; nor have the other agencies received similar results, because they have not done the same; and/or 'Because the activity is not consistent with an activity that the State agency requests; or because such activity is contrary to the law or standard of evidence.

The high complexity categories include any test with an automated outcome control tool which uses human intelligence algorithms for determining what you might want to test. Those which require a human intelligence evaluation such as a written or verbal test must include a written examination or question of writing in the form of a question which would allow questions which are not answered. Under the waiver, if a test contains no real benefits, or contains questions which are not answered, then you may not report the results. As a result, you may still receive a penalty. This means you are not allowed to complete the test or get a written assessment of the results. The written assessment of the results must not be completed by anyone other than you.

You can enroll your laboratory in the CLIA program by completing an application Form CMS-116 available on the CMS CLIA website or from your local State Agency. Send your completed application to the address of the local State Agency for the State in which your laboratory is located.
